Job Overview

The role involves scheduling appointments for patients with their primary care providers within a week of discharge. In cases where patients lack a primary care provider, assistance will be provided to secure one and arrange the necessary appointments within established guidelines.

Facilitate patient discharges from the Emergency Department by coordinating and scheduling follow-up appointments effectively.

Collaborate closely with primary care physicians and specialists to ensure a comprehensive care plan for consultations, procedures, tests, surgeries, or other services as prescribed by the attending ED provider.

Verify insurance coverage and update demographic information as needed.

Collect and process patient payments efficiently.

Work to retain patients within the Cooper Health System by scheduling directly in the Centricity Business system, Central Scheduling, or by contacting physician offices directly.

Arrange appointments for patients with community services when appropriate.

Demonstrate the ability to work collaboratively and communicate effectively with multidisciplinary teams on behalf of the patient.

Inform patients about billing policies and managed care referral or pre-authorization requirements.

Assist patients and their families in obtaining referrals from primary care providers to necessary specialists when required.

Provide support and counseling to patients and their families as appropriate.

Communicate with referring providers as necessary to address patient and physician needs.

Partner with patients, families, the interdisciplinary team, and community resources to deliver well-coordinated, timely, and exemplary care.

Serve as a resource for community services.

Track and report weekly on all scheduled services.

Float to other registration areas as needed to assist with patient access to Cooper services.

This position encompasses all discharged patients across various care areas.

Experience Required

A minimum of 3 years of experience in a high-volume ambulatory medical practice, emergency department, or hospital admissions is required.

Education Requirements

A high school diploma is required; some college education is preferred.
