Job Overview

AWARE Inc. is dedicated to fostering strong connections within our communities and prioritizing family support. We are currently looking for empathetic and committed individuals to fill the role of Community Resource Case Manager.

Our PACT team employs a comprehensive approach to mental health services, offering a range of support including medication management, individual and group therapy, substance use treatment, vocational rehabilitation, community resource assistance, peer support, and nursing services. We aim to empower clients to effectively manage their mental health in a supportive community environment.

Key Responsibilities:

  • Utilize AWARE's Unconditional Care Philosophy to create and execute personalized treatment plans, incorporating knowledge of behavioral, clinical, and mental health strategies, as well as skill-building techniques tailored to the emotional and behavioral needs of clients.
  • Possess at least one year of experience in mental health services or a related discipline.
  • Exhibit strong interpersonal communication skills, both oral and written, to collaborate effectively as part of a team, accurately report observations, and facilitate meetings.

Qualifications:

  • A High School Diploma or HSE is required, along with relevant work experience.
  • A Bachelor’s degree in a social work-related field, accompanied by a minimum of one year of direct experience in human services.

Compensation: Community Resource Case Managers at AWARE earn $22.19 per hour.
