Job Summary

We are seeking a highly skilled and compassionate Registered Nurse Home Health to join our team. As a Registered Nurse Home Health, you will be responsible for providing high-quality patient care in the home setting, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Provide high-quality patient care in the home setting
  • Collaborate with the interdisciplinary team to develop and implement care plans
  • Communicate effectively with patients, families, and healthcare team members
  • Document patient care and progress in a timely and accurate manner
Requirements
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
Preferred Qualifications
  • Associates Degree or Bachelor's Degree in Nursing
  • Previous Home Health or Hospice Experience
  • Previous experience with HomeCare HomeBase
