Job Title: Registered Nurse - Behavioral Health Home Care

Job Summary:

We are seeking a highly skilled and compassionate Registered Nurse to join our team of healthcare professionals. As a Registered Nurse - Behavioral Health Home Care, you will be responsible for providing high-quality care to patients in their homes, focusing on their physical, emotional, and social needs.

Key Responsibilities:

  • Assess and evaluate patients' physical, psychological, and social needs to develop personalized care plans.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Monitor patients' progress, adjust care plans as needed, and document all interactions and outcomes.
  • Provide education and support to patients and their families on disease management, self-care, and healthy lifestyle habits.

Requirements:

  • Current, unrestricted state RN license.
  • 1+ years of experience in a clinical care setting as a nurse.
  • Able and willing to travel within branch/office coverage area.
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
  • Must be able and willing to travel 50%.

Preferred Qualifications:

  • Associates Degree or Bachelors Degree in Nursing.
  • Previous Home Health or Hospice Experience.
  • Previous experience with Homecare HomeBase.
