Job Summary

Empowered for Excellence is seeking a highly motivated and compassionate Behavioral Health Specialist to join our team. As a key member of our Adult Clinical Team, you will provide behavioral health case management services, including therapeutic behavioral services, psychosocial rehabilitation, community psychiatric supportive treatment, and substance use disorder case management. This is an hourly position with a flexible schedule, and you will work under the supervision of the Director of Clinical Services in our Toledo Office.

Key Responsibilities:

  • Provide behavioral health case management services to adult clients, including assessment, planning, and implementation of treatment plans.
  • Collaborate with the Integrated Care Team to ensure seamless delivery of services and effective communication with clients and families.
  • Document all interactions with clients, including progress notes, referrals, and releases of information, in a timely and accurate manner.
  • Provide education and support to clients, family members, and peers regarding behavioral health, symptom management, and wellness.
  • Develop partnerships with referral and funding sources to ensure access to community resources and services.
  • Attend required staff meetings, trainings, and individual or group supervision as scheduled.

Requirements:

  • Current CDCA Certificate
  • Must complete trainings for status of QMHS to provide mental health services
  • High School diploma
  • Must have demonstrated experience in healthcare, rehabilitative, or other behavioral health setting
  • Active Driver's license and current auto insurance
  • Background check and Employee Drug Screen
