PACS Inc is seeking a dedicated Case Management Support Specialist to join our team.

Key Responsibilities:

  • Support the Case Manager by maintaining regular communication with residents and their families regarding treatment plans.
  • Address residents' inquiries about their care, treatment strategies, illness progression, and other concerns to ensure their comfort and security.
  • Complete necessary documentation as required.
  • Assist in coordinating care among residents, healthcare providers, financial services, and third-party payers.
  • Ensure accurate record-keeping and timely completion of required paperwork.
  • Collaborate with the Case Manager while demonstrating the ability to work independently and make sound judgments in interactions with healthcare professionals, payers, and residents and their families.
  • Effectively communicate and foster cooperation among residents, families, caregivers, physicians, nurses, and other healthcare partners.
  • Maintain professionalism and respectful interactions with residents, families, staff, and partners.
