PROGRAM DESCRIPTION:

The Differential Response program offers home visiting and case management services to families reported to Child Protective Services and assessed as low-risk. Case Managers play a crucial role in connecting families with resource centers, in-home therapy, and community services. This position involves delivering strength-based, family-centered therapeutic services to at-risk families. It includes conducting comprehensive assessments of family needs and providing individual, family, and/or group therapy to enhance family functioning.

POSITION PURPOSE AND OBJECTIVES:

The Family Support Case Manager is responsible for delivering strength-based, family-centered home visiting and case management services to at-risk families receiving Path 1 (low risk) and Path 2 (moderate risk) referrals from the Human Services Agency Child Welfare Services Department. Most families served have a child in the home screened by CPS and are referred to Differential Response to mitigate current risk factors. Case Managers conduct thorough assessments of family needs and assist in linking families to local community resources to address those needs and enhance family functioning. Case Managers will operate within San Mateo County.

ESSENTIAL JOB FUNCTIONS:
  • Provide case management and community response services for all Path 1 and Path 2 Differential Response referrals from the Human Services Agency Child Welfare Department.
  • Conduct weekly home visits for ongoing assessment, support, and referrals for families (including financial assistance, legal, physical, and mental health services).
  • Engage in weekly individual and group supervision.
  • Participate in all relevant training to enhance and support Differential Response services.
  • Collaborate with school and community-based service providers.
  • Assist caregivers in addressing practical parenting and household needs, such as budgeting and housekeeping.
  • Coordinate multiple services provided to clients by StarVista and other supporting organizations, ensuring full collaboration among all parties.
  • Maintain high-quality documentation, including progress notes using a database system, tracking all services provided to clients, and auditing charts to meet contract and quality assurance requirements.
  • Develop and maintain a structured independent work schedule, as this position involves significant autonomy.
  • Deliver safe, effective, culturally competent, and family-centered direct care in accordance with the agency's procedures and standards.
  • Perform other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
  • Strong case management skills.
  • Excellent written and verbal communication abilities.
  • Familiarity with public and private community resources.
  • Understanding of child development.
MINIMUM QUALIFICATIONS:
  • We seek an energetic and dedicated individual interested in working with families using a non-judgmental, strength-based approach.
  • Must possess a valid California Driver's license.
  • Bilingual in one of San Mateo County's threshold languages preferred.
  • Bachelor's degree in social work, psychology, counseling, education, or equivalent experience. Knowledge of communities and schools in San Mateo County, and/or experience in a community-based setting providing services to children and/or families.
