Case Navigator Specialist

The Case Navigator Specialist is a critical role at Community SeniorServ, responsible for performing duties for the Care Coordination Services, also referred to as Enhanced Care Management. This statewide Medi-Cal benefit addresses the clinical and non-clinical needs of the highest-need Medi-Cal members by building trusting relationships and providing intensive coordination of health and health-related services.
Key Responsibilities- Meet members where they are, on the street, in a shelter, in their doctor's office, or at home, to meet their needs.
- Coordinate community supports services to help address health-related social needs, such as access to healthy foods or safe housing.
- Conduct client-specific assessments of needs, identify problems, and establish client-centered immediate requirements and long-range goals.
- Arrange and coordinate a network of supportive services and entitlements consistent with mutually-developed care plans.
- Maintain required records and reports in compliance with department, agency, local, state, and federal requirements.
- Bachelor's Degree in Social Work or Social Services, Gerontology, or Health Sciences.
- Bilingual in Spanish or threshold language.
- Must have own transportation, valid California Driver's license, and auto insurance as required by state law.
- Prior experience with Care Transitions Program and Methodology, including training in the Care Transitions Intervention (SM) model developed by Eric Coleman, M.D., is a significant plus.
Community SeniorServ is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. Our mission is to combat hunger and loneliness among older adults through nutritious meals, friendly visits, and safety checks, and to support families through day services. We deliver nearly 2 million meals annually to 20,000 at-risk older adults.
