Job Description
As a Peer Support Specialist with True Team Medical, you will play a critical role in supporting clients receiving ACT team services. Your responsibilities will include:
  • Assisting in identifying, planning, scheduling, and implementing services for clients.
  • Developing transition plans when clients move in and out of services.
  • Serving as an advocate in the community.
  • Providing written documentation as required, including treatment notes, goals, and plans for the medical record.
  • Being available to respond as needed to clients and coworkers 24/7/365.
  • Demonstrating fidelity to the ACT Model as measured by TMACT.
