POSITION SUMMARY:

The Clinical Care Coordinator at Gracepoint's Adult Emergency Crisis Stabilization facility is responsible for providing advocacy, coordination, and continuity of evidence-based care for individuals and families in need. This role involves a collaborative approach to triage, assessment, planning, and communication, as part of a multidisciplinary treatment team. The position reports to the unit or department supervisor.

  • Inpatient facility with a capacity of 60 beds for adults aged 18 and above.

KEY RESPONSIBILITIES:
