Role Summary:

The RN Clinical Care Coordinator is an integral part of the care management team, collaborating with Hospitalists to enhance patient health outcomes. This role employs care management principles, patient placement strategies, and value-driven care approaches. The Coordinator will prioritize care coordination and discharge planning for patients within the Florida Medical Clinic system.

Essential to this position is the ability to engage in collaborative care coordination activities that address patient needs. The RN will utilize both in-person and telephonic communication to implement care management strategies effectively. Responsibilities include working closely with patients, families, physicians, and the broader healthcare team to ensure that patients receive the necessary support, resources, education, follow-up appointments, and home care services to improve health outcomes and minimize the risk of readmission.

This role adheres to established standards of practice and performance to achieve positive patient outcomes while ensuring compliance with all relevant federal and state regulations.

Preferred Qualifications:

  • Registered Nurse with an active Florida License
  • Minimum of 3 years of experience as a Registered Nurse
  • Familiarity with Medicare and value-based care populations, including available benefits and services
  • Experience in discharge planning and healthcare service coordination
  • Proficient in various electronic medical record (EMR) systems
  • Able to work independently, manage a caseload, and meet deadlines
  • Strong attention to detail, organizational, and time management skills
  • Excellent verbal and written communication abilities

Department Standards:

Performs departmental responsibilities efficiently and in a timely manner while maintaining patient confidentiality. Understands the role of the RN Clinical Care Coordinator and the objectives of care management.

Facilitates communication and collaboration among patients, families, and healthcare teams to ensure a seamless transition of care. Establishes comprehensive discharge plans in partnership with patients and their families upon admission.

Identifies and addresses potential barriers to care, ensuring that patients have access to necessary home care services, including nursing, therapy, and community resources.

Completes all required documentation accurately and promptly within the EMR system, while striving to meet internal goals and embodying the values and mission of Florida Medical Clinic.
