POSITION SUMMARY:
The Care Coordinator plays a crucial role in advocating for and ensuring the seamless delivery of evidence-based healthcare services to individuals and families. This position involves a collaborative approach to triage, assessment, planning, and communication within the Adult Inpatient program. The Care Coordinator is an integral member of a multidisciplinary treatment team and reports directly to the unit or department supervisor.
QUALIFICATIONS:
A Bachelor's degree in a Human Services-related field is required, along with a minimum of one year of experience in the mental health sector. Candidates must possess the ability to read, analyze, and interpret clinical records, procedures, and governmental regulations. Strong writing skills for documentation and the ability to communicate effectively with managers, patients, service providers, managed care entities, family members, and other stakeholders are essential.
PHYSICAL DEMANDS:
The role requires various physical abilities, including reaching, bending, talking, and fine hand coordination. Candidates should be capable of lifting up to 50 pounds independently and may need to complete standing presentations. Specific vision abilities required include close, distance, color, peripheral, depth perception, and the ability to adjust focus.
FUNCTIONAL RESPONSIBILITIES INCLUDE:
- Conducting comprehensive assessments and coordinating community resources tailored to patient needs as per program requirements.
- Completing all clinical and administrative documentation within established timelines.
- Maintaining effective communication and fostering cooperative relationships with both internal and external resources.
- Developing service plans, treatment plans, and aftercare plans with clear objectives in accordance with program requirements.
- Ensuring efficient workflow management and organization.
- Upholding high standards of clinical documentation quality.
- Performing additional duties as assigned or required.
FISCAL RESPONSIBILITIES INCLUDE:
- Utilizing agency resources, systems, equipment, and processes effectively and efficiently.
- Maintaining a continuous focus on cost-saving initiatives across responsibilities and deliverables.
- Employing effective time management strategies to prevent overtime and budgetary concerns.
Typical working hours are Monday through Friday, from 8:00 AM to 4:30 PM.
