Job Summary

The Home Care RN Intake Coordinator plays a vital role in ensuring seamless transitions of care from hospital to home health. As a key member of the Health First Home Care team, this individual will facilitate clinical referral reviews, care management coordination, and collaboration with referral sources to deliver exceptional healthcare outcomes.

Key Responsibilities
  • Perform individualized clinical assessments of patient medical records to ensure safe transitions of care.
  • Communicate with referral sources to gather demographics, diagnoses, and clinical information to facilitate continuity of care.
  • Utilize clinical nursing skills and judgment to identify and verify appropriateness of patients for home health level of care.
  • Serve as an education resource for referral sources, patients, and family members throughout the home health referral process.
  • Consistently communicate with stakeholders regarding referral status and final disposition.
  • Collaborate with clinical teams to coordinate home health referrals and meet monthly financial targets.
Requirements
  • Associate Degree in Nursing.
  • Minimum two years of acute care experience as an RN.
  • Current, valid State of Florida RN license or endorsement.
  • American Heart Association Basic Life Support Healthcare Provider Card upon hire and maintain.
Preferred Qualifications
  • Bachelor Degree in Nursing.
  • Minimum one year home health nursing or case management experience.
Physical Requirements

The majority of time involves sitting or standing; occasional walking, bending, stooping. Long periods of computer time or at workstation. Light work that may include lifting or moving objects up to 20 pounds with or without assistance.

May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise. Communicating with others to exchange information. Visual acuity and hand-eye coordination to perform tasks. Workspace may vary from open to confined; on site or remote. May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
