Job Overview:
The RN Case Manager is responsible for orchestrating the comprehensive interdisciplinary care plan for patients, overseeing their journey from admission through to discharge. This role serves as a vital link between patients, their families, and healthcare providers, ensuring that all necessary services are delivered efficiently and effectively. The RN Case Manager will utilize their expertise and critical thinking skills to set and achieve care objectives, demonstrating a degree of creativity and independence in their role.
Key Responsibilities:
- Coordinate and manage the overall patient care plan.
- Act as a liaison between patients, families, and healthcare teams.
- Utilize clinical experience to assess patient needs and plan appropriate interventions.
- Document and communicate patient progress and care modifications.
- Work collaboratively with healthcare professionals to ensure high-quality care delivery.
Qualifications:
- Must possess a valid nursing license.
- 2-4 years of clinical nursing experience is required.
- Familiarity with standard practices and procedures in the healthcare field.
