Job Summary

We are seeking a highly skilled Physical Therapist Assistant to join our team. As a Home Health PTA, you will travel to patients' homes to administer services and provide a variety of tasks during patient visits, depending on the specific plan of care and type of patient.

Key Responsibilities

  • Assist in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
  • Participate in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted according to company policy.
  • Perform routine strengthening and therapeutic exercises and assist patients with the use of self-help devices, as directed.
  • Instruct patients and family members on exercise programs and provide guidance on the use of self-help devices.
  • Identify potential ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and report to the PT.
  • Observe and record activities and findings in the clinical record and report to the PT and physician any adverse reaction to treatment.
  • Consult with the PT regarding the outcome of home visits and report physical findings.
  • Instruct other health team personnel, including Home Health Aides, and family members on certain phases of physical therapy, as appropriate.
  • Write daily progress notes on patients and submit according to company policy, and prepare periodic summary reports as indicated.

Requirements

  • Graduate of an accredited Physical Therapy Assistant program or equivalent education.
  • Licensed to practice as a PTA in the State of Florida.
  • Current CPR Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Minimum of one year's experience in home healthcare preferred.
  • Electronic medical record experience is preferred but not essential.
  • Meets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
