Job Summary
We are seeking a highly skilled Physical Therapist Assistant to join our team. As a Home Health PTA, you will travel to patients' homes to administer services and provide a variety of tasks during patient visits, depending on the specific plan of care and type of patient.
Key Responsibilities
- Assist in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
- Participate in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted according to company policy.
- Perform routine strengthening and therapeutic exercises and assist patients with the use of self-help devices, as directed.
- Instruct patients and family members on exercise programs and provide guidance on the use of assistive devices.
- Identify potential medication issues, adverse reactions, and report them to the PT and physician.
- Observe and record patient activities and findings in the clinical record and report to the PT and physician any adverse reactions to treatment.
- Consult with the PT regarding the outcome of home visits and report physical findings.
- Instruct other healthcare team members, including Home Health Aides, on certain phases of physical therapy, as needed.
- Write daily progress notes on patients and submit them according to company policy, and prepare periodic summary reports as indicated.
Requirements
- Graduate of an accredited Physical Therapy Assistant program or equivalent education.
- Licensed to practice as a PTA in the State of Florida.
- Current CPR Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- Minimum of one year's experience in home healthcare preferred.
- Electronic medical record experience is preferred but not essential.
- Meets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
- Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
- Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
