Home Health Physical Therapist Opportunity

We are seeking a skilled and compassionate Physical Therapist to join our team in providing high-quality home health care to patients in need. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleAs a Home Health Physical Therapist, you will be responsible for providing physical therapy services to patients in their homes, helping them to regain their strength, manage their pain, and reach their goals. You will work as part of a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, COTAs, PTAs, and SLPs, to provide comprehensive care to our patients.
Responsibilities- Provide home-based physical therapy services to patients who have lost mobility, strength, and functionality due to illness or injury
- Work with patients to develop and implement individualized plans of care to achieve their goals
- Conduct assessments, observe movements, and document progress to modify care plans as needed
- Educate patients and their families on their plan of care, therapy goals, home exercises, and self-care
- Graduate of an accredited Physical Therapist program and active PT license in Colorado
- Minimum of three years of physical therapy experience, ideally one year in home healthcare
- CPR certification (demonstration course required)
- Knowledge of state and federal home health regulations
- Good clinical judgment, strong interpersonal skills, excellent time management, and compassion
