Home Health Physical Therapist Opportunity

At Interim HealthCare, we are seeking a skilled and compassionate Physical Therapist to join our team in providing high-quality home health care services to our patients. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:- Provide personalized physical therapy services to patients in their homes, focusing on improving mobility, strength, and functionality.
- Work collaboratively with our interdisciplinary team to develop and implement patient care plans that meet their unique needs and goals.
- Conduct thorough assessments, document progress, and modify care plans as needed to ensure optimal patient outcomes.
- Educate patients and their families on proper exercises, self-care techniques, and home safety modifications to promote independence and prevent further injury.
- Graduate of an accredited Physical Therapist program and active PT license in Colorado.
- Minimum of three years of physical therapy experience, preferably in home healthcare.
- CPR certification and knowledge of state and federal home health regulations.
- Excellent clinical judgment, strong interpersonal skills, and a compassionate approach to patient care.
