As a Home Health Physical Therapist Assistant, you will work as part of a home health team to provide physical therapy to patients in their homes, under the supervision of a treating Physical Therapist.

**Key Responsibilities:**

* Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to an illness or injury
* Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient's plan of care and goals
* Assist the treating Physical Therapist in documenting exercises in patient's chart and printing out exercises for home as directed by treating Physical Therapist
* Participate in educating the patient and the family with exercises under the direct supervision of the treating Physical Therapist, and other caregivers

**Requirements:**

* Graduate of an accredited Physical Therapist Assistant Program and active PTA license in Colorado
* Minimum of one (1) year of physical therapy assistant experience preferably in a home healthcare setting
* CPR Certification (demonstration course required)
* Pass a background check and drug test (we are required to test for THC)
* Knowledge of state and federal home health regulations
* Good clinical judgement, strong interpersonal skills, dexterity, and compassion
