Home Health Physical Therapist Job Description

Location: Pueblo, Colorado

Field: Home Health Care

Status: Full time, Monday to Friday, no weekends

Pay: $100 - $125 per visit

As a Home Health Physical Therapist, you will provide personalized care to patients in their homes, helping them regain strength, manage pain, and reach their goals. You will work as part of a home health team, collaborating with RNs, LPNs, CNAs, HHAs, OTs, COTAs, PTAs, and SLPs to develop and implement patient care plans.

Responsibilities:

  • Provide home-based physical therapy to patients with mobility, strength, and functionality challenges
  • Conduct assessments, observe movements, and document progress to modify care plans as needed
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients and family members on therapy goals, home exercises, and self-care

Requirements:

  • Graduate of an accredited Physical Therapist Program and active PT license in Colorado
  • Minimum of three years of physical therapy experience, ideally one year in home healthcare
  • CPR Certification (demonstration course required)
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, excellent time management, and compassion
