As a Home Health Physical Therapist, you will work closely with our interdisciplinary team to develop and implement personalized care plans that promote optimal patient outcomes.

Key Responsibilities:
  • Provide one-on-one physical therapy services to patients in their homes, focusing on improving mobility, strength, and functionality.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Collaborate with our team of healthcare professionals to ensure seamless care coordination and communication.
  • Educate patients and their families on proper exercise techniques, self-care strategies, and disease management.
  • Document patient progress and adjust treatment plans as needed to ensure optimal outcomes.
Requirements:
  • Graduate of an accredited Physical Therapist program and active PT license in Colorado.
  • Minimum of three years of physical therapy experience, preferably in home healthcare.
  • CPR certification and knowledge of state and federal home health regulations.
  • Excellent clinical judgment, strong interpersonal skills, and a compassionate approach to patient care.
