Home Health Physical Therapist Opportunity

We are seeking a skilled and compassionate Physical Therapist to join our team in providing high-quality home health care services to patients in need. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:- Provide personalized physical therapy services to patients in their homes, focusing on regaining strength, managing pain, and achieving optimal outcomes.
- Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, COTAs, PTAs, and SLPs, to develop and implement patient care plans.
- Conduct thorough assessments, observe patient movements, and document progress to inform care plan modifications.
- Educate patients and their families on therapy goals, home exercises, and self-care strategies to promote independence and empowerment.
- Graduate of an accredited Physical Therapist program and active PT license in Colorado.
- Minimum of three years of physical therapy experience, preferably one year in home healthcare.
- CPR certification and knowledge of state and federal home health regulations.
- Excellent clinical judgment, strong interpersonal skills, and a compassionate approach to patient care.
