Key Responsibilities:
- Provide personalized physical therapy to patients with mobility, strength, and functionality challenges
- Collaborate with a multidisciplinary team to develop and implement patient care plans
- Conduct assessments, document progress, and modify care plans as needed
- Educate patients and families on therapy goals, home exercises, and self-care
Requirements:
- Graduate of an accredited Physical Therapist Program and active PT license in Colorado
- Minimum of three years of physical therapy experience, ideally one year in home healthcare
- CPR Certification (demonstration course required)
- Knowledge of state and federal home health regulations
- Strong clinical judgment, interpersonal skills, time management, and compassion
