The Client Support Coordinator is an integral part of the Adult Clinical Team. Reporting to the Director of Clinical Services, this role is essential in delivering behavioral health case management services, which may include Therapeutic Behavioral Services (TBS), Psychosocial Rehabilitation (PSR), Community Psychiatric Supportive Treatment (CPST), and/or Substance Use Disorder (SUD) case management services in various community settings tailored to the needs of our clients. This is a non-exempt hourly position, with a schedule designed to fulfill the required responsibilities.

Key Responsibilities:

  • Maintain a target of 25 billable hours weekly within a 40-hour work week.
  • Demonstrate initiative and strong organizational skills.
  • Provide necessary support for adult client services.
  • Execute responsibilities in alignment with professional ethics and established policies of Empowered For Excellence.
  • Adhere to all applicable state and federal regulations regarding protected health information.
  • Collaborate within an integrated care team approach to ensure that client-established goals are met within the treatment plan timeframe.
  • Ensure timely and comprehensive documentation in client records, including progress notes, referrals, and releases of information, to be completed within 48 hours of service delivery.
  • Educate and support clients, their families, and peers on behavioral health, symptom management, and wellness strategies.
  • Engage in community outreach to connect community needs with available resources.
  • Foster partnerships with referral and funding organizations.
  • Participate in required staff meetings, training sessions, and individual or group supervision as scheduled.
  • Facilitate and co-facilitate psychoeducational and substance use groups, including but not limited to Partial Hospitalization, Intensive Outpatient, and Day Treatment programs.
  • Deliver direct clinical services and ongoing assessments to active clients, communicating any changes to the integrated care team to ensure treatment plans are updated and appropriate interventions are implemented.
  • Provide community-based services such as assistance with benefits, medical appointments, housing, food, vocational needs, and other requirements outlined in the treatment plan.
  • Review and adhere to the agency's policies and procedures.
  • Complete mandatory training to maintain compliance with agency standards.
  • Perform additional duties as assigned by the supervisor.

Required Qualifications:

  • Current CDCA Certificate.
  • Completion of necessary training to achieve QMHS status for providing mental health services.
  • High School diploma.
  • Demonstrated experience in healthcare, rehabilitation, or behavioral health settings.
  • Valid Driver's License and current auto insurance.
  • Successful completion of background checks and drug screening.

Essential Skills and Attributes:

  • Deliver clinically appropriate services addressing the behavioral health needs of adult and/or youth clients.
  • Maintain required licensure and certifications.
  • Implement effective treatment models as directed by the agency.
  • Collaborate as part of a multidisciplinary team.
  • Engage professionally and effectively with external providers.
  • Possess knowledge of community resources and methods to access them.
  • Understand child/adolescent development, cultural competency, and behavioral health and substance use disorders/treatment and assessment.

Preferred Skills:

Familiarity with Care Logic EHR System.
