Position Overview: The Hub Clinical Care Coordinator (Hub C3) plays a pivotal role in overseeing patient placement operations across the organization, ensuring seamless admissions, transfers, and discharges.

Key Responsibilities:

  • Act as the primary liaison among nursing units, service locations, support departments, and external healthcare facilities.
  • Collaborate with physicians to facilitate optimal patient placement, focusing on effective and efficient outcomes.
  • Work in conjunction with all system departments and healthcare teams to enhance Length of Stay (LOS) performance.
  • Coordinate with Admission and Unit Clinical Care Coordinators (C3s) to manage patient flow on a 24/7 basis.
  • Assign unit and bed placements based on patient clinical assessments, staffing levels, and bed availability.
  • Ensure accurate patient status and level of care are communicated effectively across the Hub.
  • Evaluate patients' clinical presentations against established guidelines to determine admission status and expected LOS.
  • Facilitate necessary accommodations for patient care needs in collaboration with nursing units.
  • Manage patient placement activities using Care Logistics software, ensuring alignment with admission orders.
  • Provide consultation and education to medical and nursing staff regarding patient status and resource utilization.
  • Oversee the centralized transfer center for direct admits and facility transfers, coordinating essential clerical tasks.
  • Partner with ancillary and service departments to prioritize testing and procedures for improved patient flow.
  • Collaborate with the Staffing Coordinator to optimize nursing staff capacity and placement.
  • Serve as a resource for patient placement and transfer management across the organization.
  • Conduct regular assessments of bed availability and capacity in collaboration with nursing units.
  • Act as a liaison for service recovery and escalation of issues to appropriate leadership when necessary.
  • Utilize patient care resources effectively to meet staffing needs based on workload and patient ratios.
  • Engage with Unit C3s to address staffing challenges proactively.

Qualifications:

  • Minimum of three (3) years of recent acute care clinical experience.
  • Bachelor of Science in Nursing is required.
  • Ability to quickly assimilate information for sound decision-making and recognize urgent situations.
  • Certification in nursing, case management, or utilization review is preferred.
