Position Overview:

The Clinical Care Coordinator (C3) plays a pivotal role in overseeing patient placement operations across the organization, ensuring seamless admissions, transfers, and discharges.

This position acts as the central point of contact within the healthcare system, facilitating communication between nursing units, service locations, and support departments. The C3 collaborates closely with physicians to optimize patient placement, aiming for effective and efficient outcomes focused on patient care.


Key Responsibilities:

1. Coordinate with Admission and Unit Clinical Care Coordinators to manage hospital-wide patient flow, ensuring timely admissions, transfers, and discharges.

2. Assess patient clinical needs to assign appropriate unit and bed placements, ensuring alignment with care requirements and nurse staffing levels.

3. Identify and address barriers to patient flow, escalating issues as necessary to maintain operational efficiency.

4. Evaluate patient clinical presentations against established guidelines to determine admission status and target length of stay.

5. Facilitate necessary accommodations for patient care, collaborating with nursing units to meet specific needs.

6. Utilize Care Logistics software to manage patient placement processes, ensuring accurate documentation and resource allocation.

7. Adjust support resources in real-time to enhance patient flow and achieve organizational length of stay objectives.

8. Provide guidance to medical and nursing staff regarding patient status, placement, and resource utilization.

9. Oversee the centralized transfer center operations, ensuring timely pre-certification and insurance authorization for admissions.

10. Collaborate with various departments to prioritize testing and procedures, optimizing patient flow throughout the system.

11. Conduct regular assessments of bed availability and capacity in collaboration with nursing units.

12. Maintain effective communication with patients and families to enhance their experience.


Qualifications:

Required:

- Minimum of three (3) years of recent clinical experience in an acute care setting.

- Bachelor of Science in Nursing.

- Strong organizational skills with the ability to manage multiple priorities effectively.

- Excellent communication skills, both written and verbal.

Preferred:

- One (1) year of supervisory experience.

- Experience in bed management and utilization review.

Licenses and Certifications:

Required:

- Valid RN License from Virginia or a reciprocal compact state.

- AHA BLS Provider CPR certification required upon hire.

Preferred:

- Certification in nursing, case management, or utilization review.
