Clinical Care Coordinator

Role Summary:
The Clinical Care Coordinator is tasked with overseeing the efficiency of patient flow within the unit. This includes ensuring appropriate patient placement, determining status, assigning Diagnosis-Related Groups (DRG) and Target Length of Stay (TLOS), and facilitating timely patient progression. The Coordinator acts as a vital link between emergency services, surgical teams, and various clinical departments to guarantee that patient care requirements are met promptly.
Key Responsibilities:
- Collaborate with the Hub and Clinical Care Coordinators to manage patient care activities, ensuring optimal placement and progression.
- Assess patient care needs and nurse staffing ratios to prioritize bed reservations effectively.
- Identify and eliminate barriers that may hinder patient flow within the unit.
- Evaluate patients' clinical presentations against established guidelines to confirm admission status and necessary care levels.
- Work closely with Utilization Review Nurses to align clinical progression with payer requirements.
- Develop comprehensive discharge plans that address patient needs and potential readmission risks.
- Lead daily huddles with the clinical care team to address patient progression and discharge challenges.
- Utilize Care Advance software to manage patient progression and communicate updates with the Hub.
- Ensure compliance with pre-procedural requirements and facilitate effective communication among the care team.
- Oversee staffing and bed assignments, collaborating with management to address any staffing needs.
Qualifications:
Required:
- Minimum of three years of recent experience in acute care nursing.
- Bachelor of Science in Nursing (BSN) is mandatory.
- Ability to work independently and manage multiple priorities effectively.
- Strong communication skills, both written and verbal.
Preferred:
- One year of supervisory experience is highly desirable.
- Experience in utilization review.
Licenses and Certifications:
- Valid RN License from Virginia or a reciprocal compact state is required.
- AHA BLS Provider CPR certification is required upon hire.
