POSITION HIGHLIGHTS:
This is a full-time opportunity within a dynamic healthcare environment.
ROLE AND RESPONSIBILITIES:
The Registered Nurse (CAP RN 2) role may serve as an entry-level designation for new hires who meet the CAP RN 2 qualifications or may be achieved through progression from CAP RN 1.
The CAP RN 2 is responsible for delivering and coordinating clinically proficient care, which includes assessment, planning, intervention, and evaluation for a designated group of patients.
As a vital member of the Chronic Kidney Disease (CKD) healthcare team, the CAP RN 2 engages in decision-making, educational initiatives, leadership roles, and quality enhancement activities aimed at improving patient care outcomes and operational efficiency.
KEY DUTIES AND ACCOUNTABILITIES:
All tasks and responsibilities are expected to be executed in compliance with company policies, nursing standards, and relevant regulations.
Conducts essential functions under the supervision of the Clinical Manager and receives guidance from the Educator, Preceptor, or a CAP RN 2, 3, 4, or 5 during the initial employment period.
Required to fulfill CAP program criteria to maintain the CAP RN 2 designation or progress to CAP RN 3.
Engages in systematic collection and analysis of patient data before, during, and after hemodialysis treatment for assigned patients, documenting findings in the medical record, adjusting treatment plans as necessary, and notifying appropriate team members as needed.
Identifies variations in patient status and collaborates with the healthcare team for input.
Oversees and ensures the safe and effective delivery of chronic hemodialysis therapy to assigned patients.
Ensures proper laboratory collection, processing, and shipping procedures are adhered to and reschedules any missed or inadequate laboratory collections.
Ensures physician orders for assigned patients are accurately entered and acknowledged in the medical record.
Initiates or assists with emergency response protocols.
Delegates appropriate tasks to direct patient care staff, including but not limited to LVN/LPNs and Patient Care Technicians.
Monitors patient care staff for compliance with techniques and adherence to company policies and procedures.
Promotes infection control and environmental safety measures.
Assesses, collaborates, and documents the educational needs of patients and families, providing initial and ongoing education to both patients and their families.
Acts as a leader, coach, mentor, and role model for new and existing employees by exemplifying appropriate behaviors, work habits, and attitudes towards patients, colleagues, supervisors, and the organization.
As a Preceptor, fulfills the following:
Demonstrates and encourages attitudes and actions that align with the organization's Mission and Core Values.
Adheres to the schedule set by the Education Department to ensure that classroom information is effectively applied in the clinical setting.
Completes all required documentation promptly.
Maintains direct supervision of new employees until they are cleared to work independently by the Education Department and Clinical Manager.
Encourages new employees to operate independently within the framework of their training schedule, providing positive feedback and suggestions for enhancing knowledge and skills.
Performs essential functions of a designated Nurse in charge as assigned by the Clinical Manager.
Collaborates with the interdisciplinary team in assessing and developing patient care plans.
Identifies expected outcomes, documents, and updates nursing assessments and care plans for assigned patients in collaboration with the interdisciplinary team.
Makes referrals to Social Workers and Registered Dietitians as appropriate for individual patient education and counseling.
Checks the availability of emergency equipment and supplies daily as assigned.
Assists with medication tracking and inventory as assigned.
Serves as a resource for the healthcare team and participates in staff training and orientation of new staff as assigned.
Participates in all required staff meetings as scheduled.
Supports and promotes the organization's mission, vision, values, and customer service philosophy.
Provides education to staff members regarding updates to policies and procedures as directed by the Supervisor.
Participates in educational and quality improvement initiatives at the facility and area level as directed by the Supervisor.
Performs any other duties as assigned by the Supervisor.
PHYSICAL REQUIREMENTS AND WORK ENVIRONMENT:
The physical demands and environmental characteristics described here are representative of those encountered by an employee while performing the essential functions of this role.
This position involves direct patient care that frequently requires heavy lifting and patient mobility assistance. Equipment aids and/or coworkers may provide support. This role necessitates frequent prolonged periods of standing, and the employee must be capable of bending over. The employee may occasionally need to move equipment weighing up to 200 lbs. and lift chemical and water solutions weighing up to 30 lbs. to a height of 5 feet. The work environment is typical of a healthcare facility, featuring air temperature control and moderate noise levels. There may be exposure to infectious and contagious diseases/materials.
Daily responsibilities include desk work, computer tasks, and interaction with patients, facility staff, and physicians. The position may require travel to training sites or other facilities.
May be asked to perform essential functions of this position in other locations with similar physical demands and working conditions as described above.
SUPERVISION:
Assigned oversight of patient care technicians/LPNs as a Team Leader or designated Nurse in charge after fulfilling the following:
Successful completion of all education and training requirements for new employees plus
A minimum of 3 months of experience in chronic hemodialysis plus
Achieving and maintaining all CAP RN 2 level criteria.
EDUCATION:
Graduate of an accredited School of Nursing.
Current appropriate state licensure.
Must meet the practice requirements in the state of employment.
EXPERIENCE AND REQUIRED SKILLS:
Entry-level for RNs with 2 years or more of nephrology nursing experience within the last two years or BSN plus 1 year of nephrology nursing experience within the last two years.
Successful completion of education and training requirements for new employees.
Successful completion and maintenance of CPR BLS Certification.
Meet all conditions of employment including Ishihara's Color Blindness Test.
Successful completion of all annual required education/training/competencies.
Successful completion of ongoing training and education as assigned or required.
