Job Summary

We are seeking a highly skilled Case Manager to join our team at Spaulding Rehabilitation. As a Case Manager, you will play a critical role in facilitating the coordination of care for our patients, from admission through discharge. Your focus will be on achieving realistic treatment outcomes within the scope of available resources.

Key Responsibilities
  • Conduct thorough assessments of patients' health and psychosocial needs, adhering to established guidelines.
  • Develop and implement case management plans in collaboration with the interdisciplinary team, patients, and families, ensuring quality and cost-effective outcomes.
  • Promote effective communication and coordination between team members, payers, and community resources to minimize fragmentation and maximize outcomes.
  • Lead patient-centered team meetings to coordinate healthcare team members and third-party payers, achieving treatment outcomes and discharge planning goals.
  • Provide education and support to patients, families, and the interdisciplinary team on treatment options, community resources, insurance benefits, and psychosocial concerns.
  • Empower patients and families to problem-solve and achieve desired outcomes, promoting patient self-determination.
  • Ensure the appropriate use of healthcare services and allocation of resources, balancing quality of care with cost-effective care on a case-by-case basis.
  • Identify and provide access to necessary resources for discharge services, promoting a safe transition to the next level of care.
  • Develop discharge plans in collaboration with the healthcare team and family, meeting patient needs and financial requirements.
  • Provide culturally competent care, incorporating patients' cultural needs into the development of discharge plans.
  • Ensure a smooth transition from acute settings through skilled nursing stays and discharge, using established standards.
  • Document all activities and outcomes in adherence to department standards.
  • Adhere to third-party coverage guidelines for each patient situation.
  • Complete additional department, organization, or network activities as per established objectives.

Qualifications

We are seeking a candidate with a Bachelor's Degree in a related field, with a Master's Degree preferred. A minimum of 3-5 years of related experience is required, with case management, utilization review, or discharge planning experience in an acute, rehabilitation, skilled nursing, or outpatient setting. A current, unrestricted license to practice in Massachusetts in a health or human services discipline is also required.

We value candidates with knowledge of community services and healthcare systems, as well as computer proficiency in Microsoft Office applications. Certification in Case Management is preferred, along with highly developed communication and interpersonal skills, working with diverse populations.
