Care Coordinator

We are seeking a skilled Care Coordinator to join our team at Spaulding Rehabilitation. As a key member of our interdisciplinary team, you will play a vital role in facilitating the coordination of care for our patients, ensuring that they receive the highest quality of care and support throughout their treatment journey.
Key Responsibilities- Conduct thorough assessments of patients' health and psychosocial needs, identifying areas for improvement and developing personalized care plans.
- Collaborate with the interdisciplinary team, patients, and families to develop and implement effective care plans, ensuring that patients' goals and preferences are met.
- Communicate and coordinate with healthcare providers, payers, and community resources to ensure seamless care transitions and optimal outcomes.
- Provide education and support to patients and families, empowering them to take an active role in their care and make informed decisions.
- Monitor and evaluate care plans, making adjustments as needed to ensure that patients receive the best possible care.
- Bachelor's degree in a related field (Master's degree preferred).
- 3-5 years of experience in case management, utilization review, or discharge planning in an acute, rehabilitation, skilled nursing, or outpatient setting.
- Current, unrestricted license to practice in Massachusetts in a health or human services discipline.
- Knowledge of community services and healthcare systems, as well as computer proficiency and strong communication skills.
