Job Summary

We are seeking a highly skilled and experienced Care Transition Specialist, Lead to join our team at Mass General Brigham in Cambridge, Massachusetts.

This is a dynamic and challenging role that requires a strong understanding of healthcare systems, regulations, and community resources.

  • Coordinate referrals to extended care facilities, home healthcare agencies, and other community resources
  • Develop and implement care plans in collaboration with patients, families, and healthcare teams
  • Monitor and evaluate the quality of care provided by post-acute facilities and community agencies
  • Provide education and training to healthcare staff on post-acute care options and resources
  • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination

Requirements:

  • Bachelor's degree in a healthcare-related field
  • 3-5 years of experience in discharge planning, utilization review, or a related field
  • Strong knowledge of healthcare regulations, community resources, and medical terminology
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
