Job Overview

Position Summary:

The Community Health Worker (CHW) for Enhanced Care Management (ECM) plays a crucial role in promoting health and wellness within the community. Under the guidance of the Case Management Supervisor, the CHW is tasked with outreach, education, and support, ensuring systematic coordination of services and comprehensive care management for individuals. This position serves as a vital link between health services and the community, enhancing access to services and improving the quality and cultural competence of service delivery.

Compensation:

The anticipated salary range for this role is between $46,800.00 and $54,995.20 annually, which equates to an hourly rate of $21.86 to $26.44. Compensation may vary based on individual qualifications, including education, certifications, skills, and experience, as we aim to attract and retain exceptional talent.

Key Responsibilities:

  • Manage a caseload of assigned ECM members effectively.
  • Conduct outreach and educational initiatives to promote health services and wellness programs through various communication methods.
  • Deliver services to vulnerable and high-risk individuals in home and community-based settings.
  • Utilize personal experiences to advocate for and support individuals within their caseload.
  • Perform individual assessments to identify non-clinical needs and address barriers to resources and services.
  • Enhance access to health and human services through direct interactions and coordinated referrals.
  • Provide basic education on chronic health conditions and health promotion to encourage understanding and self-management.
  • Facilitate enrollment in federal, state, and local programs to improve access to healthcare and support services.
  • Act as a liaison between healthcare providers and patients to ensure effective communication and service coordination.
  • Educate community members on health topics, preventive care, and available resources.
  • Assist individuals in accessing healthcare services, including appointment scheduling and transportation arrangements.
  • Collaborate with healthcare providers, social services, and community organizations to coordinate care.
  • Document community needs, services provided, and outcomes achieved.
  • Participate in community events and health fairs to promote wellness.
  • Provide culturally competent care, respecting the diverse backgrounds of community members.
  • Attend staff meetings, training sessions, and professional development activities.
  • Maintain regular contact with members according to their care plan needs.
  • Document care management activities and encounters in the Case Management System.
  • Assist clinical and social services Case Managers with care coordination and communication.
  • Gather clinical information and coordinate post-discharge services.
  • Support the training and orientation of new staff.
  • Perform additional duties as assigned.

Qualifications:

  • High School diploma or GED required.
  • Community Health Workers (CHW) Certification is necessary.
  • A minimum of 2 years of experience in a healthcare or community health setting is preferred.
  • Experience in a managed healthcare environment is advantageous.

Knowledge and Skills:

  • Strong customer service orientation and respect for member needs.
  • Ability to collaborate effectively with professionals at all levels.
  • Sensitivity to social, cultural, language, physical, and financial differences among members.
  • Proficient problem-solving skills and the ability to propose solutions.
  • Strong organizational skills to prioritize tasks based on member needs.
  • Excellent verbal and written communication skills.
  • Proficient in using common office technology and electronic Case Management systems.
  • Commitment to maintaining confidentiality and professionalism.
