Position Summary:
The Community Health Worker (CHW) at Universal Healthcare MSO LLC plays a vital role in enhancing care management by providing outreach, education, and support. Under the guidance of the Case Management Supervisor, the CHW is tasked with systematically coordinating services and comprehensive care management for individuals in the community, promoting overall health and wellness.
Compensation:
The anticipated salary range for this position is between $46,800.00 and $54,995.20 annually, which equates to approximately $21.86 to $26.44 per hour. The final compensation may vary based on individual qualifications, including education, certifications, skills, and experience.
Key Responsibilities:
- Manage a designated caseload of ECM members effectively.
- Conduct outreach and educational initiatives to promote health services and wellness programs through various communication methods.
- Deliver services to high-risk individuals, particularly those with social needs, in home and community settings.
- Utilize personal experiences to advocate for and support individuals on their caseload.
- Perform assessments to identify non-clinical needs and address barriers to resources and services.
- Enhance access to health and human services through direct interactions and referral coordination.
- Provide basic education on chronic health conditions and promote self-management.
- Facilitate enrollment in federal, state, and local programs to improve access to healthcare and support services.
- Act as a liaison between healthcare providers and patients to ensure effective communication and service coordination.
- Educate community members on health topics and available resources.
- Assist individuals in accessing healthcare services, including appointment scheduling and transportation arrangements.
- Work collaboratively with healthcare providers and community organizations to coordinate care.
- Document community needs, services provided, and outcomes achieved.
- Participate in community events to promote wellness and engage with community members.
- Provide culturally competent care, respecting the diverse backgrounds of community members.
- Attend staff meetings and professional development activities.
- Maintain regular contact with members based on their care plan needs.
- Document care management activities in the CM System as per program protocols.
- Assist clinical and social services Case Managers with care coordination and communication.
- Support post-discharge services and ensure members are aware of follow-up care needs.
- Assist in training and orienting new staff members.
- Perform additional duties as assigned.
Qualifications:
- High School diploma or GED required.
- Certification as a Community Health Worker (CHW) is necessary.
- A minimum of 2 years of experience in a healthcare or community health environment is preferred.
- Experience in a managed healthcare setting is advantageous.
Skills and Knowledge:
- Ability to provide excellent customer service and respect the needs of members.
- Willingness to collaborate with professionals at all levels.
- Sensitivity to social, cultural, and financial differences among members.
- Strong problem-solving skills and ability to prioritize tasks.
- Excellent verbal and written communication skills.
- Proficiency in office technology and electronic Case Management systems.
