Job Summary

We are seeking a highly skilled and dedicated Early Intervention Service Coordinator to join our team at A&I Avenues. As a key member of our team, you will work closely with families of children ages 0-3 to access and navigate early intervention services. Your primary responsibility will be to provide service coordination to an assigned case load, developing and implementing Individual Family Services Plans (IFSPs) that meet the unique needs of each family.

Key Responsibilities
  • Coordinate delivery of services and supports to ensure seamless implementation of IFSPs.
  • Advise families on appropriate service options and assist in acquiring needed services and supports.
  • Collaborate with providers to ensure best practices in service delivery and resolve any issues that may arise.
  • Facilitate communication among IFSP team members and service entities to ensure effective services delivery.
  • Monitor services and ensure alignment with current client needs and IFSP goals.
  • Ensure all required documentation is completed and maintained in accordance with applicable Federal, State, and A&I Avenues rules and regulations.
Requirements
  • Bachelor's degree in human services-related field or equivalent experience.
  • Five years' work/volunteer experience with long-term care and/or disabilities programs and services.
  • Strong planning and organizational skills, with ability to work independently and prioritize tasks/goals.
  • Effective written and verbal communication skills, with ability to negotiate and problem-solve.
  • Possession of a valid driver's license and ability to meet A&I Avenues' driving requirements.
Working Environment

This is an hourly, non-exempt position, and all duties can be completed in a 40-hour work week. Overtime may not be worked without prior authorization and justification of work demands outside of published agency workflow-related time to task norms.
