About the Role

Parkland Health and Hospital System is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a vital role in ensuring the health and well-being of our patients and their families.

Key Responsibilities
  1. Psychosocial Screenings and Assessments: Conduct thorough psychosocial screenings and assessments to identify patient needs and develop effective interventions.
  2. Counseling and Support: Provide emotional, social, and financial counseling to patients and families, promoting empowerment and independence.
  3. Discharge Planning: Participate in discharge planning activities, ensuring timely and safe patient discharge, and promoting optimal utilization of patient and community resources.
  4. Communication and Collaboration: Communicate daily with the interdisciplinary team, validating discharge criteria and plans, and notifying the team of newly identified resources or changes in previously identified plans.
  5. Specialized Services: Provide intervention in cases involving child or adult abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health level II placement, and sexual assault.
  6. Documentation and Reporting: Document screening, assessment, interventions, and outcomes, ensuring effective communication and compliance with established documentation procedures.
  7. Referrals and Communications: Manage referrals and communications from various sources, ensuring optimal customer service and utilizing the Patient Support Center as needed.
  8. Knowledge and Compliance: Maintain knowledge of rules, regulations, policies, procedures, laws, and guidelines impacting the Social Work field, ensuring patient discharge plans are in compliance.
  9. Team Collaboration: Maintain positive working relationships with management, vendors, other hospitals, employees, and community agencies, ensuring effective communication and collaboration.
  10. Quality Improvement: Identify ways to improve work processes and customer satisfaction, making recommendations to supervisors and implementing changes as needed.
  11. Professional Development: Stay abreast of the latest developments, advancements, and trends in the field of Social Work, integrating acquired knowledge into daily performance.
Requirements
  • Education: Master's degree in Social Work.
  • Certification/Registration/Licensure: Must be licensed by the Texas State Board of Social Work.
  • Skills and Abilities: Excellent interpersonal and counseling skills, ability to communicate effectively with diverse populations, and working knowledge of community agencies and services.
