  • Collaborate with healthcare providers to ensure seamless patient care.
  • Develop and implement case management plans to facilitate patient discharge and transition to community resources.
  • Network with community agencies to ensure patients have access to necessary resources.
  • Maintain accurate and detailed documentation of patient interactions and care plans.
  • Identify and address ethical and legal considerations in patient care.
  • Communicate effectively with patients, families, and healthcare teams to ensure optimal care.

Requirements:

  • Master's degree in Social Work (MSW) or related field.
  • Two years of experience in clinical social work, preferably in a medical setting.
  • Strong knowledge of medical social work principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.

Preferred Qualifications:

  • Experience working with patients with cystic fibrosis or other chronic illnesses.
  • Knowledge of EPIC electronic health record system.
