Key Responsibilities:
- Provide brief therapy and mental health coordination to patients with cystic fibrosis.
- Conduct psychosocial assessments to identify patients' needs and develop effective care plans.
- Collaborate with healthcare providers to ensure seamless patient care.
- Develop and implement case management plans to facilitate patient discharge and transition to community resources.
- Network with community agencies to ensure patients have access to necessary resources.
- Maintain accurate and detailed documentation of patient interactions and care plans.
- Identify and address ethical and legal considerations in patient care.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care.
Requirements:
- Master's degree in Social Work (MSW) or related field.
- Two years of experience in clinical social work, preferably in a medical setting.
- Strong knowledge of medical social work principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
Preferred Qualifications:
- Experience working with patients with cystic fibrosis or other chronic illnesses.
- Knowledge of EPIC electronic health record system.
Security Clearance:
This position requires a background check and is subject to Texas Education Code 51.215.
