Overview

The role involves facilitating initial outreach and establishing immediate connections with individuals at elevated risk of HIV acquisition, as well as those living with HIV who are newly diagnosed or have experienced interruptions in their care. The aim is to build trust, assist in connecting to necessary healthcare services, and promote ongoing engagement in primary medical care and social support systems.

Key Responsibilities

  • Support initial outreach and linkage to care for clients diagnosed with HIV.
  • Conduct research to identify clusters of individuals who may have disengaged from care.
  • Collaborate with outreach teams and Public Health entities to provide HIV counseling and testing services.
  • Disseminate vital health information to communities at risk and those already diagnosed with HIV.
  • Establish immediate communication with high-risk individuals and those living with HIV, ensuring appropriate referrals to care services.
  • Engage with the community to offer peer support, assist in enrollment and retention in medical services, and facilitate case management and other necessary social services.
  • Maintain and report statistical data as directed by management.
  • Perform additional duties as assigned.

Qualifications

  • High school diploma or GED required.
  • A minimum of four years of relevant work experience or 60 college credit hours is preferred.
  • Preference for candidates with an Associate's degree and/or certification as a peer worker, along with at least one year of experience in patient navigation.
  • Experience with the AIDS Drug Assistance Program (ADAP) and/or Ryan White Services is advantageous.
  • Proficient in standard software applications.
  • Strong verbal and written communication skills, with the ability to build and maintain effective relationships with colleagues and the public.
  • A valid Texas Driver License with a clean driving record is required.
  • Willingness to travel to various work locations as needed.
