MCM & Associates LLC is in search of a caring and skilled RN Care Coordinator to become part of our committed team, providing hospice and palliative care services.

Role Summary

The registered nurse is responsible for planning, organizing, and overseeing hospice care, with a strong focus on community health education and experience. This role involves leveraging community resources to tailor services that address the unique needs of patients and their families in their own environments.

Key Responsibilities

  • Conduct initial, comprehensive, and ongoing assessments of patients and their families to identify hospice care requirements.
  • Perform thorough physical assessments and document the history of current and past health issues.
  • Evaluate patient status by developing and initiating a care plan, regularly reassessing needs, and adjusting the care plan as necessary.
  • Create a care strategy that sets achievable goals based on nursing diagnoses, involving both the patient and family in the planning process.
  • Administer prescribed medications and treatments in accordance with the physician's care plan.
  • Provide counseling and health care guidance to patients as indicated by assessments and care plans.
  • Ensure the availability of necessary equipment and services for patient care.
  • Assist patients with daily living activities, promoting self-sufficiency and optimal comfort.
  • Manage an assigned caseload under supervision.
  • Document and maintain accurate clinical notes regarding patient conditions and care provided, including pain and symptom management outcomes.
  • Communicate effectively with physicians regarding patient needs and changes in conditions, obtaining necessary orders as required.
  • Participate in interdisciplinary team meetings.
  • Perform additional duties as assigned.

Qualifications

  • Current RN License to practice in the state of California (mandatory).
  • Graduate from a nursing program accredited by the National League for Nursing.
  • Current CPR certification (mandatory).
  • Valid driver's license and insured vehicle (mandatory).
  • At least one year of professional nursing experience within the last three years, or a baccalaureate degree in nursing from an accredited program with a current RN license.
  • Preferred: Minimum of two years of experience, with at least one year in public health, home care, or hospice nursing.
  • Management experience is not required, but the role includes supervising hospice aides.
  • Exemplary observation, problem-solving, and communication skills, both verbal and written.
  • Ability to independently organize and prioritize workload and document in a timely manner.
  • Availability to work at least one weekend per month.
  • Physical ability to perform nursing tasks, including lifting, positioning, and transferring patients, as well as handling supplies and equipment.

Job Type: Full-time

Compensation: $56.00 per hour
