Position Overview
The Registered Nurse Case Coordinator plays a vital role in our healthcare delivery system, focusing on the management of care for our members within the California Health Plan. Candidates are expected to reside in California, specifically in regions such as Sacramento County, LA County, San Diego County, San Bernardino, or Riverside County. Bilingual Spanish skills are highly valued.
Key Responsibilities
- Conduct comprehensive assessments to identify members who may benefit from case management services based on clinical evaluations and health changes.
- Develop and implement personalized case management plans in collaboration with members, caregivers, and healthcare professionals to meet individual needs and objectives.
- Perform in-person visits as necessary to ensure effective care coordination.
- Monitor and evaluate the effectiveness of care plans, documenting interventions and progress towards goals.
- Maintain a consistent case load for regular outreach and management of member needs.
- Facilitate interdisciplinary team meetings to enhance collaboration and service integration for members.
- Utilize motivational interviewing techniques to support and encourage members during interactions.
- Identify barriers to care and provide necessary coordination and assistance to address these challenges.
- Local travel may be required to visit members within a defined radius.
- Provide guidance and support to non-RN case managers as needed.
- Conduct medication reconciliation for members with complex medication regimens.
Qualifications
Education: Must be a graduate from an accredited nursing program. A Bachelor's Degree in Nursing is preferred.
Experience: A minimum of 1-3 years in case management, disease management, or relevant healthcare settings is required.
Licensure: An active and unrestricted Registered Nursing (RN) license in good standing is mandatory. A valid driver's license with a clean driving record is also required.
Work Environment: This position requires a home office setup with high-speed internet connectivity. Candidates must provide their own office furniture.
Work Schedule: Standard hours are Monday through Friday, 8:30 AM to 5:30 PM Pacific Time.
