Position Overview

Key Responsibilities

  • Conduct comprehensive assessments of patients and their families to identify hospice care needs.
  • Perform thorough physical evaluations and document medical histories.
  • Develop and implement individualized care plans, regularly reassessing and adjusting as necessary.
  • Administer prescribed medications and treatments, ensuring adherence to care plans.
  • Provide health education and counseling to patients and families based on assessments.
  • Coordinate necessary equipment and services to facilitate patient care.
  • Assist patients with daily living activities, promoting self-sufficiency and comfort.
  • Manage a designated caseload, ensuring all clinical documentation is accurate and timely.
  • Communicate effectively with physicians regarding patient conditions and care needs.
  • Participate in interdisciplinary team meetings to enhance patient care.
  • Perform additional duties as assigned to support the team and patient care objectives.
Qualifications
  • Valid RN License in the state of California.
  • Graduate from an accredited nursing program.
  • Current CPR certification.
  • Possession of a valid driver's license and insured vehicle.
  • Minimum of one year of nursing experience within the last three years, or a baccalaureate degree in nursing with an active RN license.
  • Preferred: At least two years of experience in public health, home care, or hospice nursing.
  • Strong observational, problem-solving, and communication skills.
  • Ability to prioritize and manage workload independently.
  • Availability to work at least one weekend per month.
  • Physical capability to perform nursing duties, including lifting and transferring patients.

Compensation: $56.00 per hour
