Key Responsibilities- Provide high-quality nursing care to a group of hospice patients, visiting them in their homes, assisted living facilities, and hospitals.
- Complete hospice admissions and patient visits as needed, utilizing strong clinical and communication skills.
- Answer calls from patients, families, and customers, providing timely and compassionate support.
- Make nursing visits for symptom management, deaths, and admissions as needed, working closely with patients and their families.
- Collaborate with a multidisciplinary team, including nurses, social workers, chaplains, volunteers, and CNA leadership, to ensure seamless care coordination.
- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice with at least one year of experience under license.
- Admissions and on-call experience preferred.
- Excellent time management skills, with the ability to prioritize and triage multiple concerns.
- Ability to learn and utilize an electronic medical records system.
- Reliable transportation, a valid driver's license, and auto insurance, to travel across the service area.
Suncrest Hospice is a Community Health Accreditation Partner (CHAP) certified hospice, committed to providing exceptional care and service to our patients and their families. We prioritize work-life balance, offering a competitive pay range of $40.00-$55.00 per hour, a benefits package including medical, dental, and vision insurance, and a 401k plan with employer match and 100% vesting after 90 days of employment.
