Responsibilities
  • Provide professional nursing services under the direction of a RN, adhering to state and national standards of care.
  • Develop and implement individualized care plans, focusing on patient-centered goals and outcomes.
  • Conduct thorough assessments, identify patient needs, and coordinate care with other healthcare professionals.
  • Administer medications, treatments, and therapies as prescribed, ensuring patient safety and well-being.
  • Communicate effectively with patients, families, and healthcare teams, promoting open and respectful relationships.
  • Participate in care coordination activities, discharge planning, and patient education, ensuring a smooth transition to community-based care.
Requirements
  • Graduate of an accredited LPN program or equivalent.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
