Job Summary

We are seeking a compassionate and skilled LPN to join our Hospice Inpatient Unit team. As a LPN, you will play a vital role in providing high-quality care to patients and their families during their end-of-life journey.

Key Responsibilities
  • Deliver patient-centered care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation.
  • Collaborate with interdisciplinary team members to ensure comprehensive care and support for patients and families.
  • Provide education and support to patients and families on end-of-life care and hospice services.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Registered LPN with the State of Ohio without any board actions.
  • One year of previous nurse experience.
  • CPR certification.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid Driver's License with Safe Driving Record.
  • State Minimum Automobile Insurance Coverage.
