Role Overview:
As a Clinical Program Supervisor, you will:

Lead a collaborative, multidisciplinary healthcare team, including community health workers, nurses, care coordination specialists, and licensed clinical social workers.

Oversee daily operations of the care team, including the assignment of caseloads, ensuring compliance with contracts and regulations, and driving continuous quality improvement initiatives while maintaining optimal productivity and efficiency without sacrificing quality.

Provide subject matter expertise and guidance under the direction of senior leadership, supervising all care team personnel.

Conduct regular one-on-one meetings with team members, develop individual growth plans, perform performance evaluations, and implement corrective actions as necessary while maintaining accurate documentation.

Establish accountability among staff to achieve performance benchmarks, monitor operational and financial metrics, conduct monthly case audits, and implement necessary performance improvement processes.

Facilitate ongoing education and competency assessments for care team members.

Ensure adherence to policies, procedures, and regulatory standards, and guarantee timely submission of all required documentation.

Coordinate and oversee daily huddles, interdisciplinary team meetings, and primary care provider huddles.

Develop and implement care management policies and procedures in collaboration with internal and external stakeholders to enhance program effectiveness and efficiency.

Maintain comprehensive documentation of departmental activities and prepare necessary reports, including those related to Quality Improvement initiatives.

A Day in the Life:
This position operates in a HIPAA-compliant remote work environment.

Occasional extended hours may be required to meet tight deadlines, and the ability to work under pressure is essential.

Flexibility in adjusting schedules based on operational needs is a valued trait for this role.

Qualifications:
To be considered, candidates should possess:
  • A current unrestricted RN license in Hawaii (candidates able to obtain a license quickly will be considered).
  • 1-3 years of clinical experience.
  • 3-5 years of management or leadership experience.
  • A valid driver's license and access to an insured vehicle.
Preferred Skills:
We value candidates with experience in:
  • Ambulatory settings or care management.
  • NCQA standards.
