About the Role

We are seeking a highly skilled Nurse Practitioner to join our Palliative Care team in Panama City, FL. As a key member of our interdisciplinary team, you will provide high-quality patient care, working collaboratively with our Palliative Care Medical Director and other healthcare professionals to deliver exceptional patient outcomes.

Responsibilities
  • Assess and manage the physical and psychosocial needs of patients referred to palliative care
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare teams
  • Provide education and support to patients and families, promoting self-management and empowerment
  • Participate in interdisciplinary team meetings, contributing to the development of comprehensive care plans
  • Collaborate with healthcare providers to ensure seamless transitions of care
Requirements
  • Master's degree in Nursing from an accredited institution
  • Minimum of 2 years of experience as a Nurse Practitioner in palliative care or a related field
  • Current licensure as a Registered Nurse and Nurse Practitioner
  • Board certification in a relevant specialty (e.g., ACHPN)
  • Collaborative agreement with a supervising physician
