Key Responsibilities- Provide direct patient care in accordance with established policies and procedures.
- Implement and monitor patient care plans, ensuring effective coordination of care.
- Monitor, record, and communicate patient condition as appropriate, collaborating with the healthcare team to ensure seamless care.
- Serve as a primary coordinator of all disciplines, ensuring well-coordinated patient care.
- Assess and coordinate patient discharge planning needs with members of the healthcare team.
- BLS Provider certification required.
- Licensure as a Registered Nurse in Florida or Alabama, depending on the location.
- Diploma from an accredited school of nursing or required professional licensure at time of hire.
- Experience in a similar role or setting.
