Job Summary:
We are seeking a skilled Nurse Practitioner to join our team at Ascension Sacred Heart. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our multidisciplinary team to deliver exceptional patient outcomes.
Responsibilities:
- Provide direct patient care in accordance with established policies, procedures, and protocols of the healthcare organization.
- Implement and monitor patient care plans to ensure optimal patient outcomes.
- Coordinate patient care with members of the healthcare team to ensure seamless transitions and effective communication.
- Assess and coordinate patient discharge planning needs to ensure a smooth transition to post-acute care.
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
Requirements:
- Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
- Credentialed from State Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
