About the Role

We are seeking a skilled Nurse Practitioner to join our team at Monogram Health. As a key member of our integrated Care Team, you will play a vital role in delivering personalized medical care to patients with chronic kidney and end-stage renal disease.

Responsibilities
  • Conduct comprehensive annual wellness exams and provide counseling and education to patients and families about benefits and programs available to improve their health.
  • Document patient information, including chief complaint, diagnosis, medical history, and treatment plans.
  • Coordinate care with primary care providers, specialists, and ancillary services to ensure seamless patient care.
  • Prescribe medications, order tests, and collaborate with patients' Monogram physicians to deliver evidence-based care.
  • Participate in integrated care team meetings and contribute to the development of treatment plans that prioritize patient health and satisfaction.
Requirements
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
  • Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
  • Current and unrestricted DEA certificate.
  • Ability to work autonomously and practice without direct supervision.
  • Proficiency with medical instruments and equipment, as well as computer-based data management programs and information systems.
