Envision Home Health & Hospice is a dedicated organization focused on enhancing community health through compassionate care.

We are seeking an Intake Coordinator to manage the admission processes for new patients. This role is essential in ensuring that our services are accessible and that the appropriate clinical teams are assigned to deliver care effectively.

Key Responsibilities:

  • Oversee the entire patient intake process, ensuring clear communication with patients and their families.
  • Handle referrals from healthcare providers and initiate the intake procedures.
  • Perform data entry and provide exceptional customer service while fostering positive relationships with clients and referral sources.
  • Address customer inquiries and concerns promptly.
  • Contact physician offices, hospitals, or other referral sources to gather necessary demographic and insurance information.
  • Manage the verification and authorization of insurance coverage.
  • Ensure adherence to all state and federal regulations related to patient intake.
  • Utilize electronic medical records to access and update patient information efficiently.
  • Share critical patient information with the care team to facilitate coordinated care.
  • Obtain necessary authorizations for service requests from non-Medicare payers.
  • Collaborate with the Marketing and Clinical Director to secure required documentation from physicians.
  • Assist in the orientation and training of new staff members.

Qualifications:

  • High school diploma or GED required.
  • Preferred: 2 years of administrative experience in a healthcare setting.
  • Familiarity with the Washington State area is advantageous.
  • Minimum of one year of administrative experience in Senior Living, Assisted Living, Memory Care, Hospice, or Home Health is required.
  • Experience in hospice referrals/intake is highly desirable.
  • Proficient computer skills, particularly in electronic intake systems and phone triage.
  • Strong communication skills, a positive demeanor, and a collaborative spirit.
  • Exceptional organizational skills with the ability to manage multiple tasks effectively.
  • Commitment to delivering high-quality patient care and excellent customer service.
  • Knowledge of medical terminology is beneficial.
