Patient Intake Specialist

Envision Home Health & Hospice is dedicated to providing exceptional care to our community. We are currently seeking a Patient Intake Specialist to join our team on a PRN basis. This position is essential for ensuring a smooth admission process for new patients and can be performed remotely or from our office.
This role is currently PRN, with the potential to transition to Part-Time.
Key Responsibilities:
- Oversee the entire patient intake process, including direct communication with patients and their families.
- Receive referrals from healthcare providers and initiate the intake process.
- Perform data entry and maintain positive relationships with clients and referral sources.
- Address customer inquiries and concerns promptly.
- Contact physician offices and other referral sources to gather necessary demographic and insurance information.
- Manage the verification and authorization of insurance benefits.
- Ensure compliance with all relevant state and federal regulations regarding referrals and intake.
- Utilize electronic medical records to manage and input patient data effectively.
- Share critical patient information with the care team to facilitate coordinated care.
- Obtain necessary authorizations for service requests from non-Medicare payers.
- Collaborate with the Marketing and Clinical Director to secure required documentation from physicians.
- Assist in training and orienting new team members.
Qualifications:
- High school diploma or GED required.
- Minimum of 2 years of administrative experience in healthcare preferred.
- At least 1 year of administrative experience in healthcare is required.
- Experience in Senior Living, Assisted Living, Memory Care, Hospice, or Home Health is essential.
- Hospice experience, particularly in referrals and intake, is highly desirable.
- Proficient computer skills, including electronic intake and phone triage.
- Strong communication skills, a positive attitude, and a collaborative spirit.
- Highly organized with the ability to manage multiple tasks efficiently.
- Exceptional customer service skills and a commitment to high-quality patient care.
- Familiarity with medical terminology is a plus.
